This was originally in a formal script format - here it is a kind of mixture of formats.
CU = Close Up

1. EXT. DARK SEEDY ALLEY EVENING
LONG SHOT ZOOMS IN TO:
JACK moves along the alley like a man with something to hide.
CUT TO MCU
He glances at his watch as he hurries towards a dingy doorway.
CUT TO CU OF SIGN
Above the door a neon sign flickers and spits. It reads "Introductions". A neon rose adorns its tackiness. Jack stop in front of the doorway and begins to look up.
CUT TO POV LOOKING UP - FOLLOWS UP STAIRS
JACK looks up at the sign then heads inside and up the stairs.
2. INT. SEEDY OFFICE EVENING
POV SHOT ARRIVING AT THE TOP OF THE STAIRS
DORIS is waiting for him at the top of the stairs
JACK Hello (MUTTERED GUILTILY)
DORIS My Mister Black so nice to see you again. Please come this way. (SMOOTH AND PROFESSIONAL)
DORIS gestures towards an office doorway.
CUT TO JACK MOVING TOWARDS THE DOOR PAST DORIS
JACK walks towards the door fiddling with his tie.
CUT TO MCU He looks down at the tie and tries to scrape off some leftover lunch as he walks in the door.
3. INT ADJOINING OFFICE EVENING
POV SHOT FROM POV OF MELANIE SITTING IN A CHAIR LOOKING TOWARDS JACK
JACK walks into the room, dropping his tie back down to his corpulent chest.
POV SHOT FROM JACK - ECU OF MELANIE'S CLEAVAGE AND FOLLOWS ACTION
JACK looks at the MELANIE's generous cleavage.
MELANIE Hello
JACK's eyes run down her body looking at her hips and thighs.
MELANIE (LAUGHS) Hello
JACK's eyes travel upward again to MELANIE's neck and up to meet her eyes. Only there are no eyes - there is no head.
ZOOM IN ON BLOODY NECK STUMP
CUT TO 2 SHOT MELANIE's head is under her arm.
JACK'S eyes are bugging out of his head.
CUT TO MCU OF MELANIE'S HEAD
MELANIE looks up at him and winks.
MELANIE How you doing, big boy?
CUT TO MCU OF JACK
JACK backs away towards the door, his eyes nearly bursting out of his head, his jaw slack, inarticulate noises coming from his throat.
JACK Arp. Urp. Nahh.
He turns and runs out the door.
4. INT. SEEDY OFFICE NIGHT
CUT TO 2 SHOT OF DORIS AND JACK
DORIS watches as JACK stares wildly at her and then runs down the stairs.
DORIS walks to the door laughing.
DORIS Good Riddance!
DORIS slams the door after him.
DORIS walks to the office door.
DORIS Thanks Mel. You were great as always.
MELANIE replaces her head.
MELANIE (smiling) He won't be back again.
FADE TO BLACK
